TCP My Student’s Toolbox is a helpful tool which lists details about the participating child, what tends to “set the child off,” as well as their strengths and what they are responsive to. This sheet is very useful as a reference document for schools working with children having graduated from the Transitional Care Program.
TCP Referral Form is useful for listing important information regarding the child and the treatment process, including contact information, family situation, and details on treatment such as what worked, what didn’t, and what the child still needs to work on when returning to the community school.
